 Dental Benefits Minimize

Delta Dental of Colorado is the dental provider for the City and County of Denver. Delta offers three plans through the City and County of Denver:

  • Exclusive Panel Option (EPO) Group #6791
  • Delta Preferred Provider Option II (PPO) High Group #6793
  • Delta Preferred Provider Option III (PPO) Low Group #6026

    Delta Dental does not issue personalized ID cards. To obtain services employees may either present their provider with the group # (above) for their plan or print off an ID card by clicking on the group # (above).

     Exclusive Panel Options

  • Employees pay only a copay for covered services.
  • Participating dentists submit claims directly to Delta Dental
  • Benefits for the EPO plan are available only when services are provided by a Delta Dental PPO Panel Dentists.
  • Go to Delta Dental’s web site to find a participating dentist. EPO dentists can be found by clicking on "Delta Dental PPO" under product selection.
  • Plan highlights include: $10 exams, fixed cost for all other procedures and no annual out-of-pocket maximum.

  • Delta Preferred Provider Option II (PPO) High

  • Employees have the option of obtaining services from a PPO (in-network) dentist or a non-PPO (non participating or Delta Premier network dentist).
  • Go to Delta Dental’s web site to find a participating dentist. PPO dentists can be found by clicking on "Delta Dental PPO" under product selection. Non-PPO dentists can be found by clicking on "Delta Dental Premier".
  • Employees who elect to use a non-PPO dentist may incur higher out-of-pocket costs.
  • In-network benefits include 100% reimbursement for exams, 90% reimbursement for basic services (such as fillings), 60% reimbursement for major services (such as crowns)and a $2000 annual maximum benefit.
  • Employees who elect to utilize non-PPO dentists will be responsible for higher out-of-pocket costs.

  • Delta Preferred Provider Option III (PPO) Low

  • Employees have the option of obtaining services from a PPO (in-network) dentist or a non-PPO (non participating or Delta Premier network dentist).
  • Go to Delta Dental’s web web site to find a participating dentist. PPO dentists can be found by clicking on "Delta Dental PPO" under product selection. Non-PPO dentists can be found by clicking on "Delta Dental Premier".
  • Employees who elect to use a non-PPO dentist may incur higher out-of-pocket costs.
  • In-network benefits include 100% reimbursement for exams, 80% reimbursement for basic services (such as fillings), 50% reimbursement for major services (such as crowns) and a $1250 annual maximum benefit.
  • Employees who elect to utilize non-PPO dentists will be responsible for higher out-of-pocket costs.
