Additional Employee/Dependent Life and AD&D Plans

Additional Life Plans
Employees may elect Additional Life coverage and Spouse life coverage in units of $5,000 to a maximum benefit of $300,000. New employees may elect up to $100,000 for themselves and $30,000 for their spouses if they apply within 30 days of employment without being subject to medical underwriting approval. Late applicants and request for coverage increase are subject to medical underwriting approval.
Any employee may elect Additional Accidental Death and Dismemberment (AD&D) for themselves and their family in increments of $5,000 up to $500,000 at any time.
Any employee may elect dependent life insurance for $5,000 or 10,000 for their dependent children.
