Enrollment Information and Forms for New Employees
Health Plans
New employees may enroll in the medical, dental, vision, short-term disability, flex cash medical and dependent care plans within 30 days of their hire date. Employees must complete the Generic Enrollment Form and submit to their agency HR/Payroll department prior to the end of the 30 day enrollment period. If the new employee does not enroll during their new hire period, they will not be allowed to enroll in the above coverages until Open Enrollment or until the occurrence of a major Life Event (see below).
Basic Life
The City and County of Denver provides a basic life insurance policy to all CSA employees. The policy is equal to 2 times the employee's annual salary to a maximum benefit of $100,000 for full time employees. Employees are asked complete the beneficiary designation form and submit to their agency HR/Payroll department.

Open Enrollment
Existing employees may only change their medical, dental, vision, short-term disability, flex cash medical, dependent care and Hyatt Legal plan elections during open enrollment or if they experience a major life event (family status change.) Open enrollment occurs each October with a January 1 effective date.
Major Life Events
Some examples of major life events include: birth, adoption, death, marriage, divorce, hours change or loss of other coverage. If employees experience a major life event which impacts their desired enrollments, they are responsible for contacting their payroll technician within 30 days of the occurrence. Documentation is required. If employees do not notify the Career Service Benefit department or their agency payroll department within 30 days, they will not be allowed to modify/change their benefit package. This could result in lack of coverage for any new dependents.
