 My Tax Forms Minimize
W-4 Forms
Employees are required to complete a W-4 form so that we can calculate your federal tax withholdings.
 
If you have access to the city network, you can view and update your W-4 information online through the following steps: 
  1. Go to the DOT homepage and sign in with your network username and password
  2. On the "My Page" tab, select "My W-4 Tax Information"
  3. Update your tax data and submit your changes 
If you don't have access to the city network, you can drop off your updated W-4, or send it to us by mail or fax:

You should check your payslip periodically to make sure your tax information and deductions are correct, such as: 
  • New benefit deductions
  • Changes to your 457 Plan
  • Changes to your allowances
  • Changes to your dependents 
  • Changes to FSA medical amounts
  • Exempt status changes
  • If you notice any problems with your deductions, please contact us for help.
     


    W-2 Forms
    W-2s are mailed to employees on or before January 31st each year. Check that your mailing address is current to avoid any problems receiving your W-2. If you are interested in receiving your Form W-2 up to two weeks sooner, check out how to receive your electronic Form W-2 that you can print yourself. You must have access to DOT to sign up for electronic W-2s.

    If you need to request a new W2, please complete this Request W2 Form and submit to CitywideAdmin.Help@denvergov.org. If you are requesting your 2011 W2, Citywide will begin processing these requests beginning February 15, 2012. 
     


    W-5 Forms
    The W-5 Advanced Earned Income Credit Form was repealed. Please speak to your tax consultant for additional information.

    Social Security Tax Limits for 2012
    • Maximum Wage Base: $110,000.00
    • Rate Withheld (Employee): 4.2%
      • Maximum Tax Withheld: $110,000.00 x 4.2% = $4,620.00
    • Rate Withheld (Employer): 6.2%
      • Maximum Tax Withheld: $106,800.00 x 6.2% = $6,621.60


    Medicare Tax for 2012
    • Wage Base: There is no maximum wage base or deduction for Medicare Tax
    • Rate: 1.45%


    457 Plan and 457 Plan Catch-up Contribution Limits for 2012
    • 457 Plan Contribution Limit: $17,000.00
    • 457 Plan Catch-Up Contribution Limit: $22,500.00 (for employees 50 years or older)
    To calculate the percentage:
    Take the maximum contribution allowed – divided by your estimated year-to-date earning, equals the per pay period 457 Plan percentage (i.e., $17,000.00/$45,000.00 = 38%)

    Denver Employees Retirement Plan (DERP)

     "Making Work Pay" tax credit ended Minimize

    Please be aware that in 2009 and 2010, most wage earners benefited from the changes made to the federal income tax withholding tables to implement the "Making Work Pay" tax credit. This credit ended in December of 2010 and therefore, has been updated in PeopleSoft. As of the 2/27/2011, you may see a difference in your taxes due to this update. If you have specific questions about how this may apply to you, please consult with your tax professional. You may also get additional information about the "Making Work Pay" tax credit by visiting the IRS website at www.irs.gov and doing a search on Making Work Pay.

     If you claim Exempt Minimize
    If you claim “exempt” from withholding, you must submit a new W-4 form each year, no later than February 15th, if you do not, your exemptions will automatically be set to “Single” and “0”.
